Investigation Form

Have we asked you to complete an e-mail questionnaire?

This part of our website is especially for potential witnesses in specific cases to send to us information that could assist our investigation. Not all of our investigations use this method of information collection. It depends on the circumstances of each case. There may be times when there are no cases using this process. But if you have been contacted invited by us to provide some details about your dealings with a particular business or person, the following notes will guide you.

You will have received either a letter or an e-mail from one of our investigators asking for your help with information. The investigator will have advised you of the name of the case and the purpose of the online questionnaire.

Once you have completed the questionnaire, or as much of it as you can, simply use the "submit" button on the questionnaire to send it to us. There is no compulsion on you to help but your assistance will be greatly appreciated. We may not need to contact you any further. However your contribution could prove to be significant and it is possible that we may have to contact you again. Click on the case name below if it applies to you.

Reporting a fraud

However, if you have visited this website, not because of a letter from an investigator, but to alert us to a new suspected fraud, the questionnaires on this page are not for you.
